I participate in many insurance plans, including Aetna, Anthem, BCBS, HUSKY/Medicaid, as well as out-of-network with UHC/Oxford and Cigna. If your plan is not listed here, we can usually work within an out-of-network benefit. Contact your customer service representative for more information about this. The number is usually available on the back of your insurance card.
Co-pays or other fees are expected at the time of service, and can be made in cash, check, debit, or credit card. A sliding scale is available for the uninsured.
Pay for Services
